DETAILS: - Role will be at our newly built outpatient facility starting in Sept 2025 - Connected to our independent living facility - Patient-focused, relationship-driven care model - Support staff in place to help you with all other duties - Weekly physician visits and easy access to physicians via phone - Open to full or part-time providers - Experience working with geriatric patients as an NP for 3+ years required
