  • Physical Therapist (PT) position in Livonia, Michigan.
  • Occasional weekends (1-2 days per month max).
  • May work some holidays (at least one).
  • Use of EPIC system.
  • Serving Wayne and Washtenaw Counties.

    Job Requirements

  • American Heart Association BLS certification required for all direct patient care personnel.
  • State license/certification verification must be dated within 30 days of start.
  • Fit Test required.
  • Trinity 10 Panel Drug Screen required within 30 days of start date.
  • Trinity Code of Conduct Form required.
  • Trinity I9 form required.
  • Confidentiality and System Security Agreement required.
  • Integrity and Audit Services Form required.
  • Trinity Specialty Specific Competency Testing must be completed within a year.
  • Color Vision test required.
  • COVID-19 vaccine proof or declination required.
  • MMR vaccination proof or IgG titer required.
  • Hepatitis B vaccination series required; titers not accepted.
  • Influenza vaccine proof or declination required.
  • Driver's License/State ID copy required.
  • 2 Step PPD OR IGRA (Quantiferon/Tspot) required; positive TB requires chest x-ray.
  • Varicella vaccination proof or IgG titer required.
  • Physical/Health Statement from exam within last year required.
  • Trinity Profile including resume, skills checklist, and references required.
  • Conflict Of Interest Disclosure Form required.
  • Criminal Background Check including SSN verification required, dated within 30 days of start.
  • Education Verification required.
  • EPLS report required, dated within past 30 days.
  • Fingerprinting required (state and department specific).
  • GSA/OIG exclusion report required, dated within past 30 days.
  • Joint Commission CORE Exam within a year required.
  • NSO report required, dated within 30 days of start.
  • System Access Form must be completed and uploaded one week before start.
  • Tdap vaccine or declination required (declination accepted if no pediatric interaction).
  • Trinity Skills Checklist required for clinical contractors.
